When I create a new project I only have 2003 add-ins and 2007 add-ins, no workbook option under office.

VSTO 2005 SE includes the Add-ins project for 2003 and 2007 while VSTO 2005 includes the workbook projects. VSTO 2005 is not available for free so You need to buy a license in order to work with workbook level solutions.

As an alternative You can consider to download and install the present beta version of VSTO 2008 (aka 'Orca') but it only works with Office 2007.

Is Office installed on the development computer If it is not, only the application-level addin projects appear in VS. For more information, you can read the section titled "Microsoft Office Requirements for Developing Solutions" at http://msdn2.microsoft.com/en-us/library/54ds2za4(VS.90).aspx

Visual Studio Tools for Office in Orcas does have support for both Office 2003 and the 2007 Microsoft Office system. A list of supported features is available at http://msdn2.microsoft.com/en-us/library/aa942839(VS.90).aspx
